A trip to the vet is in order, yes, but only for an assessment at this
point.

Some cats are born intersex, having both male and female anatomy to some
degree. Or the cat may just have testicles on the small side. Only an exam
be a vet can give you a definitive answer.

If the cat is sterile, then there would be no point in having him neutered
for purposes of population control.

retained testicles is linked with a higher risk of cancer - removing them is
best for the health of the cat. A sterile cat will still exhibit the
unpleasant behaviors associated with hormones (spraying, etc). Neutering will
be better for the health of the cat and the sanity of the owners - just do it!

Sounds like he was neutered young and never aquired one of the
secondary sex characteristic of males cats -- the big jowls.
